If you decide to take your husband’s last name, here’s an easy to follow checklist to make sure you don’t miss anything.
IMPORTANT: Do NOT change your name before the honeymoon. Your name has to match on all travel documents, including your driver’s license and/or passport. Processing a name change on a passport typically takes anywhere from four to six weeks.
Get Your Marriage License. This is step one. Before you can change your name on anything, you’ll need the original, certified (with a raised seal) marriage license from the county clerk. This should automatically be mailed to you when the license is filed, but if you haven’t received it, or need additional copies, call the clerk’s office where it was filed.
Change your Social Security Card. Fill out the form on the SSA website (www.socialsecurity.gov/forms/ss-5.pdf) and mail it, along with a copy of your certified marriage license, to your local Social Security Administration office (https://secure.ssa.gov/ICON/main.jsp). You’ll keep the same number, but should receive a new card in about 10 days with your new name.
Change your Driver’s License. You’ll have to block of a morning or afternoon (or, in some cases, a morning AND an afternoon), to take a trip to your local DMV or place of license issuance to get one with your new last name. Bring every form of identification you can possibly get your hands on – your old license, your marriage license, your new social security card, utility bills showing your name and address, just to make sure you have all your bases covered. You do NOT want to have to go through this process again.
Change your Bank Accounts. The best – and fastest – way to do this is to go to a local branch with your new driver’s license and your marriage license. Be sure to request new checks and new debit/credit cards.
All the Rest. Once you have your new social security card and driver’s license, changing your name on other items should be pretty straight forward. Some places only need a phone call, while others want to see a copy of the marriage certificate, so be sure you know the requirements of each one you’re trying to change.
